Welcome to Palo Alto Field Hockey Club. All members need to pay PAFHC dues, be a member of USA Field Hockey (formerly USFHA) and sign the Bay Area Field Hockey Association (BAFHA) waiver.  Please access the links below for all the forms you need.

2011 Dues:
PAFHC Fees : $125
Students: $75
Set of Shirts (if needed) $60

• USA Field Hockey Membership: Renew or Join ONLINE!! You will get your confirmation immediately. Forward your email to Yad Dhami, PAFHC Treasurer, or bring him a printed copy for certification. PLEASE REMOVE YOUR PASSWORD WHEN YOU FORWARD!
• BAFHA Waiver Online release for playing in the Bay Area Field Hockey League. YOU WILL NEED TO HAVE YOUR USFHA number and exp date!
• Palo Alto Online Membership Form Fill out online and submit. Upon submission of form you will get a printout for submitting your dues via check.

Please submit your forms + dues by APRIL 3 to be included in the roster. Send the hard copy forms to:
